filed for extenstion, what forms do I need to fill out to get it finished?

You would use the same forms that you would have used if you had filed on time.

Just file your tax return as usual. Same forms as always.

Same forms as before. Get a tax program and turn on the tutor or the part of the program that just asks you the questions and you fill in info for every yes. Turbo Tax is reliable and fairly simple to follow. Any questions, you will have to look up at. Or just go get a professional and let them do it the first time so that you have some idea how to handle it in the years to come. Your extension is only good for a short time and you need to get going. . .
